Hazel A. Straka, 85, died on Friday evening, Feb. 16 surrounded by her loving family at Upland Hills Nursing and Rehab in Dodgeville.

Funeral services will be held at 11 a.m. on Tuesday, Feb. 20 at St. Thomas Catholic Church in Montfort with burial to follow at the church cemetery. Visitation will be from 3–8 p.m. on Monday, Feb. 19 at the Soman–Larson Funeral Home in Montfort, where a prayer service will be held at 2:30 p.m. on Monday.
