Lavern K. Konichek, 67, Muscoda and formerly of Richland Center, passed away April 16, 2021 at UW Hospital in Madison. 
Graveside services will be held at 2 p.m. on Wednesday, April 28, 2021 at the Evergreen Cemetery in Prairie du Chien.
