Mary Smith (nee Lambert), age 68, of West Bend/Hartford, passed away peacefully at home surrounded by her family on Tuesday, Dec. 9, 2014. Mary was born on Nov. 8, 1946 in Menomonie, to Richard and Jane (nee Smallman) Lambert.

A Memorial Service was held at Phillip Funeral Home on Monday, Dec. 15, 2014 at 2:15 p.m. with Father David LaPlante of St. Killian Congregation officiating. In lieu of flowers donations can be made to Cedar Community Hospice or the Creutzfeldt-Jakob Foundation.

The family would like to extend a special thank you to the Cedar Community Hospice staff, especially Rachel, Carol and Shelly, who provided our wife and mother with exceptional care and treated us like family.  Also thank you to all of the family and friends who provided love, support, meals and prayers during this difficult time.
