Phyllis M. Boak, 94, Boscobel, died on Tuesday, June 21, 2016 at Boscobel Care and Rehab.

Services will be held on Monday, June 27, 2016 at 1 p.m. at the Beeman-Patchak Funeral Home in Boscobel, with burial in the Boscobel Cemetery.
