Susan M. Prochaska, 64, of Muscoda, died on Friday, June 17, 2016 at UW Hospital, Madison.

Mass of Christian Burial was held Wednesday, June 22, 2016 at St. John the Baptist Catholic Church, Muscoda. Father Garrett Kau officiated. Burial was in the church cemetery.

Memorials may be made to the Susan M. Prochaska Memorial Fund.
