Hazel I. Coulthard, 89, of Cuba City, WI passed away on Saturday, October, 15, 2011 at the Southwest Health Center Nursing Home in Cuba City, WI.
Services will be held at 10:30 am on Tuesday, October 18, at the St. Rose of Lima Catholic Church, Cuba City, WI with Father David Flanagan officiating. Burial will be at the St. Rose of Lima Cemetery, Cuba City, WI. Friends may call from 4pm-7pm on Monday, October17 at the Haudenshield Funeral Home & Cremation Services, in Cuba City, WI and on Tuesday, October 18, from 9am till time of service at the funeral home.
