Jerry F. “Cookie” Cook, 74, Platteville, died on Tuesday, May 14 at Gray’s Nursing Home in Platteville.

Funeral services were at 3 p.m. on Sunday, May 19 at the Melby Funeral Home and Crematory, Platteville. Pastor Mary Ann Floerke officiated. Burial was at Hillside Cemetery in Platteville, where military rites were accorded by American Legion Post #42 and VFW Post #5274. Visitation was from 4–8 p.m. on Saturday at the Melby Funeral Home and Crematory and from 2 p.m. until the time of the service on Sunday at the funeral home.
